Family 'tormented' by Sutton Coldfield man's death after cancer operation delay
Ravinder Lally, a 39-year-old Sutton Coldfield man with ulcerative colitis, died of advanced rectal cancer after delays in further surgery following an initial operation in 2023. An NHS investigation identified an inappropriate decision to remove part of the bowel while leaving the rectum, inadequate contingency planning during the surgeon’s absence, and poor documentation, while stating that it could not establish that the delays directly caused his death. An inquest recorded death from natural causes, with the coroner unable to conclude that delayed rectal removal was more likely than not to have contributed to the death. Lally’s family remains distressed and says he was not given sufficient information about the surgical risks. University Hospitals Birmingham has introduced new guidance and other colorectal-care changes in response to the findings.

Behind the classroom doors of Sandwell's first 'exceptional' school
Moat Farm Junior School in Sandwell received an exceptional rating for behaviour and attendance under Ofsted’s new inspection framework, becoming the first school in the borough to do so. Leaders credit strong community support, a focus on safety and engagement, and extensive enrichment activities for the achievement. Pupils describe valuable experiences through trips, leadership roles and academic support, particularly for disadvantaged and SEND students. The accomplishment reflects broader improvements in Sandwell’s education system after years of underperformance, signalling sustained progress across the borough’s schools.

Traders back plan to ban masks in Stafford town centre
Traders in Stafford support proposed rules allowing police to require removal of face coverings suspected of being used to conceal identities during antisocial behaviour. The measures, part of a Public Spaces Protection Order now under consultation, would carry a £100 penalty for refusal with exemptions for religious, cultural and medical reasons. Local businesses and officials cite rising intimidation, disorder and shoplifting, while police highlight ongoing patrols and dispersal powers. Community leaders argue that prevention and youth engagement are key to reducing issues. Similar measures are already in place in several other UK areas.
