‘No malicious intent’ in Eljamel document destruction, report finds
An independent AAB investigation found no malicious intent in NHS Tayside employees’ destruction of about 40 theatre logbooks connected to disgraced neurosurgeon Sam Eljamel, but concluded that the records should not have been destroyed. The report identified inadequate governance, unclear responsibilities, ineffective escalation and systemic failures in communication and information management, despite a 2024 order from inquiry chairman Lord Weir not to destroy relevant documents. NHS Tayside accepted all findings, apologised to affected patients and families, and published an action plan addressing 15 recommendations, including stronger oversight and clearer procedures for preserving records subject to legal requirements.

Amazon apologises to woman not allowed to bring baby to Fife course
Amazon apologised after entrepreneur Rachael Bews was unable to attend a business course at its Fife fulfilment centre because she was told she could not bring her breastfed baby to the site. Bews had planned for her husband to care for the child elsewhere on the campus while she left sessions to breastfeed, but was informed during travel that children under six were not permitted on-site for safety reasons. Amazon said the policy had not been clearly communicated and invited her to a future event. Bews called for more accessibility in leadership programmes for breastfeeding parents and carers.
