Andrew Cox, a 34-year-old prisoner at HMP Guys Marsh, died on December 2, 2023, from sudden death associated with polydrug use after reportedly going unseen for 24 hours. A Prisons and Probation Ombudsman investigation found repeated failures by staff to conduct cell checks, investigate an obstructed observation panel and respond to warnings about Cox’s drug activities and suspected drone deliveries. The report described the failures as shocking and said additional staff were needed to support an effective drug strategy at the prison.
