Probe into Death Linked to Extreme Vitamin Deficiency
A Norfolk coroner will investigate the treatment given to 66-year-old Angela June Willgoss at James Paget University Hospital before her death following a cardiac arrest linked to severe vitamin deficiency. The inquest will examine whether very low vitamin D results were communicated to clinicians, how low calcium was treated, whether appropriate cardiac monitoring was provided, and whether any acts or omissions contributed to her death. The hospital’s patient safety investigation report, clinical evidence, NICE guidance and evidence of remedial actions will form part of the case. A one-day full inquest is scheduled for November 24, 2026.

Retired Swaffham Cook Drowned at Home, Inquest Hears
A provisional medical cause of death for 69-year-old retired cook Paula Ann Hensby was given as drowning due to alcohol intoxication after she died at her home in Swaffham. Area coroner Maeve Sykes formally opened the inquest and requested reports from Norfolk Police, the deceased’s GP, Change Grow Live and Norfolk County Council adult social services, along with a family pen portrait. A full inquest is scheduled for August 18 at Norfolk Coroner’s Court in Norwich.
