Woman fed dog food by care workers who 'couldn't read label' before her death
A Portsmouth woman known as Mrs Y was given three mouthfuls of dog food by a Radis care worker after staff failed to read its packaging. A Local Government and Social Care Ombudsman investigation found wider failings, including prolonged periods without supervision, missed or falsified records, inadequate food and poor overnight care. Mrs Y, who had limited mobility and required a soft diet after a mini-stroke, died in July 2024 from frailty of old age. Portsmouth City Council accepted the findings, apologised, ended its contract with Radis and was advised to reimburse the family and make a £1,000 payment for distress and investigative delays.
