Young dad, 22, found dead in his cell got drugs behind bars with 'apparent ease'
A Prisons and Probation Ombudsman investigation found that Dylan Woodhead, 22, obtained psychoactive substances with apparent ease while held at HMP Hindley and was not properly checked during evening and morning roll calls before his death in January 2021. The report said prison staff missed opportunities to address his suicide and self-harm risk, although it could not establish that the failures caused his death. An inquest later concluded that Woodhead died by suicide. Prison authorities accepted the recommendations and cited expanded security measures to tackle drug smuggling.

‘Determined’ teacher found dead was ‘under stress ahead of Ofsted visit’
A 38‑year‑old teacher, Katie Allen‑Gyves, died from the combined effects of prescription and over‑the‑counter medication while experiencing significant work‑related stress ahead of an expected Ofsted inspection. An inquest found no indication of intent to self‑harm and concluded the death was a tragic accident. Evidence highlighted her long hours, sleep difficulties, rising blood pressure and concerns she shared with her GP but did not convey to her school. Colleagues described her as gifted, dedicated and well‑liked, while her family noted her increasing workload and exhaustion. The coroner emphasised the pressures faced by teachers and recognised Allen‑Gyves’ commitment and personal progress.
